3. Will you agree to be fully objective and report the results of your assessment without regard to the district’s policies or recommendations?
4. Are you able to provide evaluative and regular services virtually?
5. Are you willing to provide me with a copy of your CV and rate card?
6. Do you have an evaluator/provider team under you? If so, are they all SE certified and otherwise appropriately credentialed?
7. Do you have prior experience evaluating and addressing the educational needs of a student with.... [DESCRIBE YOUR CHILD’S STRENGTHS AND WEAKNESSES HERE]?
8. As part of the total cost of the testing and report, are you willing to:
a. Participate in a CSE/IEP/504/School Meeting to review and explain your report?
b. Do a comprehensive review/comparative analysis of all IEPs and District testing, along with other private testing (both educational and therapeutic)?
c. Request test protocols from the school staff who have previously administered testing to the student and analyze the data provided?
d. Observe the student in the current classroom? Observe in the home?
e. Opine on the type of program and services necessary now? Specifically:
i. Teacher/student ratio of classroom/program
ii. Student Profile (IQs, reading and math needs, classifications, management needs, etc.)
iii. Methodologies needed
iv. Training for teacher or related service providers needed
v. Nature and level of related services (frequency, duration, methodology, training)
f. Make placement recommendations (specific schools or programs)?
g. Opine on what, if anything, is needed to make up for lost time?
